Incident Reports

We encourage every department to have an incident report form to use in the event of an accident or incident that requires documentation. Below is a generic incident report form that you may wish to use if you do not have your own. Also included are incident report forms from other departments that you may wish to use as a model for your own.

In addition, we have included injury-reporting protocol for university insurance filing and for sending injured parties to Patient's First Medical Centers in town if the injury is not of emergency nature.
